Individual Community Living

Bundled service that includes six service categories. ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.

ICLS covers assistance and support for eligible people 65 years and older enrolled in Elderly Waiver (EW) or the Alternative Care (AC) program.

Active Cognitive Support

This service category focuses on assisting individuals experiencing cognitive challenges by addressing their specific concerns and promoting mental well-being. Providers may offer problem-solving support for daily living concerns, deliver verbal reassurance, and observe and redirect behaviors related to orientation or cognition. Both in-person and remote check-ins can be conducted to identify emerging issues and help resolve them promptly.

Adaptive Support Service

Adaptive Support Services are designed to encourage self-sufficiency by helping individuals adopt strategies that reduce dependence on human assistance. Providers may offer verbal, visual, or tactile guidance to help complete tasks, introduce and demonstrate cues or reminder tools (such as calendars or lists), assist with understanding instructions for assistive technologies, and reinforce strategies that promote ongoing independence.

Activities of Daily Living (ADLs)

This category addresses the individual's needs related to basic daily living activities. Providers can offer reminders or cues to initiate ADLs and give intermittent physical assistance with dressing, grooming, eating, toileting, mobility, transferring, and positioning. As needed, they may also offer continuous supervision and hands-on support for tasks such as bathing.

Household Management

Household Management services help individuals maintain a well-organized and functional living environment. Providers can assist with routine chores such as cleaning, meal planning and preparation, and shopping for essential household and personal items. They may also support budgeting, money management, and communication tasks (e.g., handling mail, using email, placing phone calls, or scheduling appointments). Transportation may be provided when it is essential to achieving household management goals and when other options are unavailable.

Health, Safety, and Wellness

These services aim to promote the individual’s overall health and safety. Providers can identify changes in health status and communicate concerns to case managers or caregivers. They may coordinate safety modifications in the home, provide reminders or assistance with exercises and wellness routines, and support medication management, including reminders, administration, or refills. They may also operate medical devices or adaptive technology as instructed by licensed health professionals and monitor health conditions, reporting any significant changes accordingly.

Community Engagement

Community Engagement services support individuals in participating meaningfully in community life. Providers assist with accessing local activities, services, and resources that foster inclusion and social interaction. They help build and sustain informal support networks and can offer transportation when necessary for achieving engagement goals—particularly when public or informal transit options are not available.
