Residential Habilitation services support individuals with disabilities in living meaningful, independent lives within their communities. People ages 18 and older
In-home Family Supports
In-home Family Supports are tailored to assist individuals and their families within the home environment. These services aim to enable individuals to remain at home or return after time away by empowering both the person and their family members. Support may include training to enhance caregiving skills, direct assistance with daily tasks, or help with integrating into community life. The focus is on building family capacity, reducing stress, and supporting the individual’s development and independence within their home setting.
People ages 18 and older
These services apply to adults living in provider-controlled settings such as group homes, supervised apartments, or foster care. Each setting supports a person-centered lifestyle while offering tailored assistance for daily living, emotional well-being, and social engagement. Individuals may live in community residential settings (CRS), townhomes, or other shared living spaces. The focus remains on maintaining or building essential skills including hygiene, cooking, socialization, money management, and behavior improvement — all while respecting the individual’s autonomy and goals.
Supported living services (SLS)
Supported Living Services are for individuals who require consistent, daily support due to behavioral challenges, medical conditions, or physical and life skill needs. For children under 18, SLS supports those living outside their biological or adoptive homes. The service ensures 24/7 supervision and help in personal care, mobility, communication, and behavior management. Providers work closely with individuals to create supportive environments that encourage participation, skill development, and increased autonomy.
