We provide personalized in-home medical visits, helping patients recover safely and comfortably in familiar surroundings.

Residential Rehabilitation

Residential Habilitation services support individuals with disabilities in living meaningful, independent lives within their communities. These services are designed to promote and maintain physical, intellectual, emotional, and social functioning. Whether provided in the individual's home or in supported living environments, the goal is to empower each person with the skills needed to manage everyday tasks and participate fully in community life.

Core to Residential Habilitation is assistance with Activities of Daily Living (ADLs) such as cooking, hygiene, and cleaning, alongside broader goals like improving communication skills, community integration, and emotional wellbeing. The support provided is person-centered, ensuring that every individual receives the care that best meets their personal goals, preferences, and living situations.

In-home Family Supports

In-home Family Supports are tailored to assist individuals and their families within the home environment. These services aim to enable individuals to remain at home or return after time away by empowering both the person and their family members. Support may include training to enhance caregiving skills, direct assistance with daily tasks, or help with integrating into community life. The focus is on building family capacity, reducing stress, and supporting the individual’s development and independence within their home setting.

People ages 18 and older

These services apply to adults living in provider-controlled settings such as group homes, supervised apartments, or foster care. Each setting supports a person-centered lifestyle while offering tailored assistance for daily living, emotional well-being, and social engagement. Individuals may live in community residential settings (CRS), townhomes, or other shared living spaces. The focus remains on maintaining or building essential skills including hygiene, cooking, socialization, money management, and behavior improvement — all while respecting the individual’s autonomy and goals.

Supported living services (SLS)

Supported Living Services are for individuals who require consistent, daily support due to behavioral challenges, medical conditions, or physical and life skill needs. For children under 18, SLS supports those living outside their biological or adoptive homes. The service ensures 24/7 supervision and help in personal care, mobility, communication, and behavior management. Providers work closely with individuals to create supportive environments that encourage participation, skill development, and increased autonomy.
Provider-Controlled Settings for Adults
Adults (18+) may receive SLS in specialized settings such as community residential services (CRS), family foster homes, supervised living facilities, or provider-managed housing like apartments or townhomes. The focus is on supporting everyday life skills, personal development, and integration into the community. The environment is designed for safety, structure, and the promotion of autonomy, balancing supervision with freedom.
Children Living Outside the Biological/Adoptive Home
Children requiring care outside their biological or adoptive home may receive SLS in structured environments with trained caregivers. These settings provide daily support for managing behaviors, medical routines, education, and life skills. The services are child-focused, aiming to create a stable, nurturing setting that supports healthy development, emotional security, and eventual reintegration or transition into adulthood.
