OUR MISSION
The State of Minnesota has a significant population of citizens 65 and above, as well as people living with disabilities and other challenges. The mission of Zion Care Services, and its skilled team, is to complement the efforts of the state and federal government to provide some essential support to the elderly and people living with disabilities to facilitate their integration into the community, support them in their efforts towards independent living and ensure they live in dignity as valuable members of the community.
About Zion Care Services
Zion Care Services LLC is a service provider registered in Minnesota to provide Home and Community-Based Services to people 65 and over, people living with disabilities and people who need rehabilitation and integration into the community.
Zion Care Services is licensed to provide Independent Living Skills Training. Individual Community Living Skills, Rehabilitation Support, and Employment Services, with a goal to maintain normal relationships with their families and friends, live independent lives, engage in employment, and participate in their various communities.

Waiver and Alternative Care Programs.
ZCS provides its services under the Waiver and Alternative Care Programs. Waiver and Alternative Care (AC) programs provide home and community-based services (HCBS) to meet the needs of people with disabilities and older adults.
Examples of such programs are:
Developmental Disabilities
For people with developmental disabilities or related conditions who require the level of care provided in an intermediate care facility for persons with developmental disabilities (ICF/DD)
Alternative Care
For older Minnesotans who require the level of care provided in a nursing facility and who are not yet eligible for Medical Assistance (MA)
Community Alternative Care (CAC)
For people who are chronically ill or medically fragile and require the level of care provided in a hospital
Brain Injury (BI)
For people with a traumatic, acquired or degenerative brain injury who require the level of care provided in a nursing facility that provides specialized services for people with BI, or who require the level of care provided in a neurobehavioral hospital
Community Access for Disability Inclusion (CADI)
For people with disabilities who require the level of care provided in a nursing facility
Elderly Waiver
For older Minnesotans who require the level of care provided in a nursing facility.
